Myofascial pain syndrome in the pelvic area

Myofascial pain syndrome (MFBS) in the pelvic region is characterized by the presence of trigger points (TrPs) in the pelvic floor (PF) muscles and pain that is localized directly in the pelvis but simultaneously radiates to surrounding structures. This pain is often difficult to distinguish from internal complaints, which are often mimiced or accompanied by the pain. This bachelor's thesis addresses the subject of pain in the pelvic region and TrPs, which are related to myofascial structures, particularly PD. In the theoretical part, it aims to bring together the existing knowledge on MFBS with a narrower focus on the pelvic region and to present a comprehensive examination procedure and treatment options. The examination of PD in MFBS should be comprehensive, including history, palpation, dynamic tests and pelvic region specific tests. Therapy is initiated with manual techniques (MT), includes therapeutic physical education (TPE) methods to integrate the pelvis into global movement patterns. Training awareness of PD and the ability to relax it play an important role as well. The psychological side of the person should not be neglected, as pelvic difficulties are related to the intimate parts, in many cases reducing the quality of life and limiting the performance of daily activities. Trigger points problems in physiotherapy

My bachelor thesis deals with trigger points (MTrPs) problems in physiotherapy and their therapy. The aims of the thesis was to map the trigger points problems and clarify possibilities oh therapy these points. In the theoretical part of the bachelor thesis I describe division, causes, and mechanism of origin, manifestations and consequences, therapy, presence and ways of chaining, referred pain of trigger points and head-forward posture. For the practical part of the work, I chose the method of qualitive research. There were 10 probands in the research, for which I looked for trigger points and then based on theoretical knowledge, I compared the presence of trigger point patterns in various symptoms of the probands. Then I treated 4 of the selected probands with these points. During therapies, I combined manual techniques, thermotherapy and physical therapy. In my opinion, the topic of trigger point is still open and some questins are not answered yet. The subject of research is still the mechanism of MTrPs. The most frequetly mentioned theory is the Integrated hypothesis. There are other hypothesis that are not sufficiently scientifically based. Ways to chain of MTrPs and ways or mechanism of referred pain are others not entirely explanatory chapters. According to theories of several authors, reffered pain is manifested by activity of nervous system at the level of the spinal cord. Based on researches by various authors related to trigger point therapies, the following conclusions can be drawn. When comparing ischemic compression and the trigger point pressure release technique, it is better to use the pressure release technique. When comparing manual techniques with dry needle application, it can be said that both techniques have a comparable effect. However, the use of tape should be combined with another technique, as the tape acts rather locally. After treatment of MTrPs in probands, MTrPs reformed due to poor movement stereotypes. Therefore, in my opinion, it is necessary to pay attention to the reeducation of movement patterns such as poor posture.

Objectives: The main focus of this thesis is to compare the effect of treatment using radial shock wave and selected manual techniques. Methods: In this diploma thesis, the Wagner FDIX Force digital pressure algometer was first used to evaluate the effectiveness of both experimental methods to examine the pressure pain threshold, then a Visual Analogue Scale for pain and a two-arm goniometer to examine range of motion to the lateroflexion of cervical spine to opposite side from the occurring MTrp. The outcome data was stored in Microsoft Excel 365. Basic descriptive statistic was used for data analysis using mean values, medians, standard deviations, etc. Due to the heterogeneity of input values in both groups, a regression model and nonparametric Mann-Whitney U-test was used. The significance level for statistical tests was set at α = 0.05. Name: The Effect of Facilitating Kinesio Taping of lower scapula fixators on pain threshold of trigger points in the m. trapezius pars descendens Objectives: This study is a pilot study. The goal of this study is to determine effects of Facilitating Kinesio Taping of lower scapula fixators on painfulness of trigger points in the m. trapezius pars descendens. Methods: The study took place in a private medical office with thirteen patients. All of these patients were physically examined using standard tests of physiotherapy. Patients were measured using a pressure algometer to assess trigger points as painful. They reported the intensity of pain on VAS. The places of measurement were trigger points one and two in the muscle according Travell and Simons. Data were analyzed using the statistical program SPSS 15.0 and Microsoft Office Excel. Results: This study had three hypotheses. In the case of the first hypothesis, I concluded that KT has an effect on the pain threshold and its application will reduce the pain threshold in m.trapezius pars descendens. Comparison of physical therapy of trigger points in the upper portion of the trapezius muscle using ultrasonotherapy and laser phototherapy

The thesis is divided into theoretical and practical part. The theoretical part contains information about anatomy of the trapezius muscle and its myofascial pathology. Author further discusses trigger points, their histopathology and diagnostic and therapeutic capabilities. A significant section of the theoretical part explains the physical principle of ultrasound therapy and laser phototherapy, including clinical aspects and their use in the treatment of trigger points. The practical part is focused on comparing the efficacy and relative effectiveness of two methods of physical therapy - ultrasound therapy and laser phototherapy, where myofascial trigger points treatment effect can be expected. Research group is made up of 47 patients divided into two groups. These patients have a doctor confirmed trigger point in the upper portion of trapezius muscle. The first group of patients is treated by exactly parametrically defined ultrasound therapy. The second group of patients is treated by specifically defined laser phototherapy. The aim of this thesis is to compare the above mentioned physical procedures in terms of subjective and objective efficacy in the treatment of trigger points and also to compare their relative effectiveness.
Autonomic nervous system and its relationship to functional disorders of the muscle

The aim of this paper was to summarize the current knowledge of function and regulation of autonomic nervous system (ANS). We wanted to assess possible involvement of ANS in changes of muscle tension. Sympathetic nervous system affects the sensitivity of somatosenzory afferention, thus effecting the function of whole nervous system. Exists evidence about ANS effect on cutaneous afferents activity as well as muscle spindle afferent activity. Sympathetic modulation of proprioceptive information from muscle spindles may influence α motoneuron excitability. ANS may affect muscle tonus regulation. The selective activation of trigger point (TRP) during heightened muscle sympathetic efferent activity (MSNA) was proved. The influence of manual therapy on visual acuity.

Title of thesis: The effect of manual therapy on visual acuity. Goal: The main goal is to present theoretical facts about manual eye therapy, to examine the effect of manual therapy of oculomotor muscles on selected parameters: visual acuity, subjective evaluation using a questionnaire and eye muscle pain in patients with refractive errors and eye accommodation disorders. Methods: The research was completed by 20 patients with refractive errors and eye accommodation disorders, from aged 18 to 55 years. An initial and subsequent treatment and exit examination and of eye muscles with the help of manual techniques, in particular, reciprocal inhibition and mobilization of eyeball were performed. The initial examination was performed by an optometrist, who assessed visual acuity by using an Autorefractometr, Snellen optotype and Red-green test test. After than was performed the subjective evaluation questionnaire, then was performed palpation assessment of attachments of eye muscles. Palpation pain proband evaluated using the Numeric Pain Rating Scale.The resulting data were processed using statistical software R. A paired t-test was used to calculate p-values. The application of combination therapy in the treatment of cervical and lumbar spine pain

This bachelor thesis deals with the effect of combination therapy (simultaneous application of electrotherapy and therapeutic ultrasound) in the treatment of vertebral algic syndrome in the cervical spine and lower back with a significant functional component (trigger points) without radicular symptoms. Furthermore, the effect of the treatment is compared with the effectiveness of transcutaneous electroneurostimulation. The theoretical part of this bachelor thesis is devoted to the description of the vertebral algic syndrome and the used treatment methods. The research part is focused on verifying the effects of combination therapy and transcutaneous electroneurostimulation, using each method in combination with individual therapeutic exercise. A total of twenty adult participants were included in the study, ten of whom were treated with combination therapy and ten were treated with transcutaneous electroneurostimulation. The effects of the therapies have been evaluated using selected pain questionnaire methods (visual analog scale, short form of the McGill Pain Questionnaire, Neck Disability Index, Oswestry Disability Index) and combination therapy intended for diagnostic purposes. The objective of this thesis is to provide a comprehensive summary about the topic of myofascial trigger points, which are considered to be the most common cause of myofascial pain and dry needling, which presents one of the therapy options. A part of the topic summary is dedicated to the Czech legislation concerning the application of dry needling in the physiotherapy practice. The main objective of this thesis is to create a systematic research of clinical studies, which compare the efficacy of dry needling to standard methods of physiotherapy. The research includes 22 randomized clinical studies, which compare the efficacy of dry needling to manual therapy, physical therapy and a combination of physiotherapy methods.
